The Evoke SmartVisit (ESV) module is a helpful tool comprised of required health assessment questions as directed by the Centers for Medicare & Medicaid Services (CMS). Each assessment question is segmented under a unique category that covers a section of the patient’s overall health profile. The ESV is designed to be conducted on a yearly basis and suffices as the documentation of an Annual Wellness Visit.
