How to Navigate to the Member ScoreCard

The member ScoreCard serves as the point of care aspect of the Evoke360 application. It is used to drive effective dialogue between the provider and patient. You may review comprehensive details of a patient’s health record by clicking through the ScoreCard sections.

A member’s ScoreCard can be accessed from the patient page, the patient banner, or through hyperlinks in various reports.

The quickest way to navigate to a member’s ScoreCard is through the patient search functionality.

1. Click the magnifying glass icon (1),
2. Type in at least three valid characters of the patient’s last name, then first name, or their identification number,
3. Click the patient’s name from the dropdown options to navigate to the patient’s page,
4. Click the ScoreCard (2) option from the left-side panel, or
5. Click the ScoreCard option in the patient banner (3) at the top of the patient page.
